Sure, the subjects' mouths are being filled with an uncomfortably sour taste that's unbearable to most for an extended period, but it's glorious watching the hilarious unique reactions they all emit, because you know that if that was you on the screen, you would have some of the same reactions. It's humorous because you know they're not in pain, just a little startled by the intense sour. The film is a documentary, but it's not a typical documentary. It just wants to document all of their reactions in a collection of hilarious clips of several subjects from young ages to adults.
I think this film could have served as an influence of several YouTube reaction channels. The YouTube channel "React" where kids, teens, YouTubers, and elders react to different YouTube videos, trends, and even sometimes try different foods or games, is a very similar medium. I love the React channel so I really enjoyed the "Sour Death Balls" film. If you watch the videos, you'll understand why my mind immediately went to thinking about the similarities between the two.
